To apply for an AIMHO Committee please do one of the following:

• Click here to submit an AIMHO Committee Application.
• Download, fill out, and email this form to jbondy@montana.edu   (Committee Application)
• Fill out, print, and submit the form to any Executive Board Member
  during the annual AIMHO Conference.   (Committee Application)
